Annual Permit Reports

2024 Annual Reports are due March 31, 2025!

Every facility with a Permit to Operate must submit together a (1) Facility Questionnaire and the (2) previous year production throughput as required in the permit’s Recordkeeping Conditions before March 31 of each year.  Please contact the District if you have additional questions.

 

1. Facility Questionnaire

Please complete "Facility Contact Update Form" (at the top of the General Permits page) when the facility’s contact information has changed to ensure the correct person is notified for any future permit or program updates. 

2. Annual Report Templates
